    SEESALT Permission Form          Don't miss out on one of the best weeks of the summer!  Print this and give this to Elley by JUNE 9.   (Rising 9 grade-high school graduate)


    Project Serve Permission Form        Take a week and share God's love with others.  Join the whole youth group as we serve in Louisville, KY.  Print this out and give it to Elley by June 30.  (Rising 7th grade-high school graduate)

    Offsite Permission Form                   Required in order for students to participate in offsite activities such as Fall Retreats, SEESALT, Mission Trips, or others..
